Spring/Summer Term Health Plan Opt-Change/Opt-Outs

If you are starting as a full-time student this Spring or Summer Term, you will automatically be enrolled in the UWSA Health Plan. You have the opportunity to add dependants or opt-out if you are already enrolled in another health insurance program.

Opt-outs and opt-changes are open May 1, 2017 and close May 12, 2017.

Opt-outs are done exclusively online here starting May 1. To add a dependant, complete an Opt-Change form on the health plan page here and submit it to the UWSA Health Plan Office.

If you were a full-time student in the Fall or Winter term, your UWSA Health Plan runs until August 31, 2017. Visit the health plan page to learn more about your coverage and how to make a claim here.
